Has anyone in the U.K. had any joy in getting a copy of the “registration of live birth” document? I was told by the head of midwifery in north Yorkshire that the form what was filled out at the hospital, which they then sent to the registry office, isnt practiced anymore... I’m waiting on a reply to “what is the practice now” ...

The head of midwifery answer is distraction...that practice was in place when you were born resulted in a piece of paper work that forms part of your health record and by law they should have it, it may not be a form but a medical record sheet stating that you were born alive and your details etc Inc mother, father, weight etc...that's the document not form you want
I'm also informed its not vital you get this bit I'm on the process of hmttuobg to get my statement of live birth which is not a form from my local authority...give as much information as possible Inc father's name. Mothers name, where you were born, date etc.. So they can find it
@John Ward thanks for that John, so where is the best place to get It? when I said form I meant document lol. I also want a copy of my 8year olds and if they don’t do it that way anymore then I’m not sure where to go, maybe the maternity records for me when I was pregnant might have it? are you creating your own ID also?
The medical record a sheet they write up minutes after you and your child were born should be in your medical records...so your local authority or local health authority should provide a copy to prove you were born alive
I looked online as I want to access same info! You can request a copy of all your medical records via your GP but they do charge ££ xx
Had an answer from my registry office! It does not deny the live birth record! I have now requested a copy of it fingers crossed I put in speech Mark's the relevant part of the reply [ please note i put in a SAR not an FOI request] "Dear Louise, Apologies for the delay in responding. In respect of your request for your live birth record, please note that section 28 of the Births and Deaths Registration Act 1953 states that the register must be kept in the safe custody of the Registrar. The original entry in the register cannot be provided to you pursuant to Schedule 2, Part 6, Paragraph 28 of the Data Protection Act 2018 as the original entry is required for archiving purposes in the public interest. Please review further guidance regarding birth certificates and the full birth certificate policy at https://www.gov.uk/government/publications/birth-certificates-and-the-full-birth-certificate-policy/birth-certificates-and-the-full-birth-certificate-policy#introduction." Fingers crossed on getting a copy of entry of live birth, it would seem they are being avoidant for a reason. Peace love and unity
Update on my birth record, after many more emails to and fro, they then asked what I wanted it for so I explained that's my business but I require it without crown copyright to it as I believe that enslaved me to a corporation I also explained I want copy of record not copy of birth certificate registration this is the response so fingers crossed. Dear Louise, I can confirm that we have the birth registered in the borough. You will need to call the contact centre so that they can take the payment. It will then be dispatched within 5 working days. This will be a certified copy of the original request signed by our Registrations Services Manager and will contain registered name of baby, father and mother's name, date of birth, occupation and address. Are you talking about your original birth certificate? With name in caps as copies have lower and upper case. I was going to apply to my local registry office? Is this something different? Should I contact local health authority where I was born and request all medical notes? From hospital not GP? Thank you for any guidance
The certificate of live birth isn’t in medical records I’ve got mine and theres wasn’t one in there. ☹️
It will he held somewhere GPs don't hold all medical records...it could be the hospital you were born at or the local community health / midwifery services who attended home birth...the local health authority should be able to shed light on this...im currently still awaiting response from the local authority re mine
